Responsible for teaching outpatient Diabetes Self-Management skills and nutritional education in class settings and/or one-to-one. In-patient basic diabetes self management and/or nutrition based on criteria established by the department.What you will do
  • Plans, prioritizes and implements educational programs which are in compliance with American Diabetes Association objectives, RDS guidelines and other identified needs. Educational Offerings are presented clearly. Completes curriculum within designated time. Performs performance improvement activities related to diabetes (minimum of one). Effectively communicates with client/family and the interdisciplinary team. Incorporates client/family rights into nursing and dietitian practice. Participates in development of DSMT, DSME, Weight management, Ask the Educator, Prediabetes, Nutrition, CGM's and CDC DPP curriculum.
  • Will perform and assess 2 or more CGM insertions/downloads. Will assist with prepump classes. Actively participates in DPP program at least once per year. Demonstrates the ability to assess, implement, and evaluate treatments and provide appropriate individualized recommendations on the needs of the patients while following departmental policy and guidelines. Consistently demonstrated evidence-based competency in the assessment of the patient and adapts care to meet the individual patient’s needs. Demonstrates professional competency. Documents in the EMR within 24 hours.
  • Highly encouraged to actively participate in diabetic community programs/health fairs including The American Diabetes Tour de Cure. One of these activities is the American Diabetes Tour de Cure. Participates in one or more EVADE meetings per year. Participates in Ask the Educator Activities. Completes social accountability report monthly. Maintains professional standards by participating in continuing education at a minimum of 15 hours annually. Seeks to meet learning needs in order to maintain competency. Attends client case conferences/staff meetings. Attends a local Certified Educator Conference yearly. Submits monthly time, and travel records according to policy and procedure.
  • Inpatient requirements for Diabetes basic self-management and nutrition basic education and documentation. Utilization of electronic records. Complete in-patient list and data search in 1 hour or less. Utilize in-patient sheets to compile notes and next day follow up's. Communication and support with nursing, providers, and team members in the provision of patient care to include collaboration about Diabetes for the specialized needs of the patient. Must understand the inpatient performance measures. Assist inpatient units, staff, and providers through teaching about hypoglycemia, hyperglycemia, A1c, and the importance of diabetes education documentation. In addition, responds to consults, High A1c’s, and newly diagnosed as priority. Demonstrates an understanding of Riverside approved educational material.
  • Demonstrates excellent service to patients/families and other customers by providing assistance to their specific needs. This includes linking patients/families with appropriate resources in the community, providing services to other customers that may be assigned to another team member. Employee is aware of community and RHS resources.
QualificationsEducation
  • Bachelors Degree, Nutrition, Dietetics or Related Field (Required) or
  • Program Graduate, Nursing (Required) and
  • High School Diploma or GED, Only Required if Nursing Diploma Graduate (Required) or
Experience
  • 2 years Dietitian or Nursing Experience (Preferred)
Licenses and Certifications
  • Registered Dietician (RD) - Commission on Dietetic Registration (CDR) Upon Hire(Required) or
  • Registered Nurse (RN) - State Department of Health Professions Licensed in State of Virginia or compact state Upon Hire(Required) and
  • Certified Diabetes Educator (CDE) - National Certification Board for Diabetes Educators Certified Diabetes Care and Educational Specialist within 2 Years(Required)
