Winchendon, Gardner, Hubbardston, New Braintree, Petersham,Templeton, Ashburnham, HardwickPOSITION SUMMARY:The Physical Therapist provides skilled physical therapy services consistent with professional and regulatory standards and policies. Participates in the development of individualized programs necessary to achieve patient care objectives.ESSENTIAL DUTIES AND RESPONSIBILITIESThe following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
  • Perform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic program. Develop a therapy plan of care to include goals and discharge plans.
  • Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition. Completes all documentation required and in accordance with state and federal regulations and policy. Demonstrate administration of objective, standardized tests and measures and performance of evidence-based practice in documentation.
  • Communicate and coordinate patient care with other members of the patient's health care team. Attend employee meetings and patient conferences.
  • Supervise physical therapy assistants in accordance with regulations and policy.
  • Provide patient and family teaching related to the patient's physical therapy program and patient/family engagement in goal achievement. Assist the patient and/or family to procure needed equipment or supplies.
  • Provide physical therapy in interventions accordance with physician's orders and accepted evidence-based professional practices.
  • Utilize appropriate hand washing, standard precautions and personal protective equipment.
  • Keep knowledge of physical therapy theory and practice current and acquires new skills as needed by attendance at workshops, courses, in-services, etc.
  • Demonstrates proficiency with the administration of objective standardized tests and measures and compliance with regulatory standards.
POSITION REQUIREMENTS:
  • Graduate of an accredited Physical Therapy program
  • Licensed as a PT in the Massachusetts
  • Home care experience, strongly preferred
  • Current CPR required
  • Current State of Residence driver's license and reliable transportation
  • Excellent organizational and interpersonal skills
  • Computer proficiency
  • Excellent verbal and written communication skills
  • Ability to read, write and speak the English language
  • Maintain required vaccinations
PHYSICAL REQUIREMENTS/WORKING CONDITIONS:
  • Work primarily indoors
  • Possible exposure to unpleasant odors
  • Possible exposure to chemicals as identified in the MSDS Manual
  • Possible exposure to infections, diseases and Bloodborne pathogens
  • Continuous exposure to patients who are ill, confused, irritable and irrational
  • Subject to hostile and emotionally upset family members, personnel and visitors
  • May work beyond normal working hours and on weekends and holidays when necessary.
