Directly provides counseling, case management and other chemical dependency services in meeting the social emotional needs of the detox/rehab patients and their family members.Responsibilities:
  • Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively.
  • Provide evaluations and psychosocial assessments for patients in collaboration with the td team.
  • Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients.
  • Prepares and keeps a current case record for each assigned patient according to HealthAlliance Hospital, OASAS, and JC standards.
  • Participate as a team member in conferences, treatment planning on behalf of patient.
  • Attend in-service training and keeps professionally updated through reading and attending seminars.
  • Provide lectures, workshop/training for patients and staff as assigned.
  • Strictly adheres to all of the confidentiality requirements imposed by law and professional conduct.
  • Observes adherence to current policies and a resource to director for program recommendations.
  • Perform any other duties as assigned.
Experience:
  • Minimum one (1) year experience providing mental health treatment preferred.
  • Experience using one or more evidence-based treatment approaches preferred.
  • Experience working with individuals with SUD preferred.
Education:
  • Masters of Social Work or Mental Health Counseling from an accredited program.
Licenses / Certifications:
  • Current NYS Masters Level licensed QHP or licensure eligible with limited permit.
