- Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively.
- Provide evaluations and psychosocial assessments for patients in collaboration with the td team.
- Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients.
- Prepares and keeps a current case record for each assigned patient according to HealthAlliance Hospital, OASAS, and JC standards.
- Participate as a team member in conferences, treatment planning on behalf of patient.
- Attend in-service training and keeps professionally updated through reading and attending seminars.
- Provide lectures, workshop/training for patients and staff as assigned.
- Strictly adheres to all of the confidentiality requirements imposed by law and professional conduct.
- Observes adherence to current policies and a resource to director for program recommendations.
- Perform any other duties as assigned.
- Minimum one (1) year experience providing mental health treatment preferred.
- Experience using one or more evidence-based treatment approaches preferred.
- Experience working with individuals with SUD preferred.
- Masters of Social Work or Mental Health Counseling from an accredited program.
- Current NYS Masters Level licensed QHP or licensure eligible with limited permit.
