Job Description:Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in New York City, Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester. We are currently hiring for a full time Social Worker for our Adult Day Health Care program at our Inwood Ave, Bronx, NY site.The Social Worker provides crisis intervention, biopsychosocial assessments, individual, family and group counseling and/or case management services to HIV positive patients, their partners, caregivers and family members, as well as other patients with issues related to chronic illness, aging, maternity, domestic violence or other social problems. Performs job duties in compliance with systems and job specific standards.ESSENTIAL FUNCTIONS of Social Worker of Adult Day Health Care:· Provides supportive counseling for a selected caseload.· Coordinates and documents community resources.· Provides a comprehensive mental health intake/assessment for all new patients referred by primary care provider or other service provider for medical and/or social/emotional issues.· Performs a mental status exam on all new Mental Health patients and reviews with psychiatrist/primary care provider.· Provides case management services as needed to support primary care.· Refers those in need of other or more intensive types of mental health and/or case management services to appropriate sources, including psychiatric evaluations.· Provides individual, group and family counseling to patients and their families.· Provides crisis intervention/emergency evaluations to patients when required.· Provides appropriate follow-up on referred patients.· Ensures program's clinical services are in compliance with grants and/or funding sources.· Develops treatment plans with patients and/or family members in a timely manner.· Responsible for the accurate and timely completion of clinical documentation in patient records.· Obtains necessary authorizations from various managed care companies for services rendered.· Demonstrates knowledge of the principles of growth and development over the life span, including identification and categorization of each patient's age-specific grouping of needs, such as those for adolescent, adult and geriatric patients.· Serve as Care manager for patients in the Impact Model.· Support integrated care through group sessions (directly an in partnership with other Providers).Required Education/Experience:Master's degree in Social Work requiredCPR/BLS required from American Red Cross or American Heart Association in person class onlySun River Health is a network of over 40 Federally Qualified Health Centers (FQHCs) providing primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually.
