What you will do
  • Maintain schedule of all Hospice services.
  • Prepare the weekend and nursing call schedule.
  • Performs nursing care and other field staff functions, as necessary.
  • Receive updated orders from physicians.
  • Takes and prepares intake referrals.
  • Coordinates care with clinical field staff.
  • Ensures that services ordered by the physician are provided in a coordinated manner.
  • Provide instruction functions as a clinical knowledge resource when needed for staff and hospice patients.
  • Participates in Interdisciplinary Group meetings when available.
  • Verifies Medicare/Medi-Cal and Insurance benefits if necessary.
  • Coordinates the use of supplies needed for the care of patients.
  • When requested, gives a report to case managers of third party payors.
Who you are:
  • Licensed Vocational Nurse licensed by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
  • At least two years of patient coordination experience or equivalent in home health, hospital or medical group.
  • Working knowledge of Medicare Conditions of Participation, and JCAHO standards.
  • Current CPR card.
  • Core competency per skills checklist.
  • Current health clearance.
