- Educates patients and families about disease process, treatment options, and potential side effects.
- Coordinate patients through the continuum of care in collaboration with the multidisciplinary team.
- Serve as the liaison between disciplines and any supportive/community services.
- Coordinate weekly multidisciplinary conferences and database tracking analysis for treatment outcome purposes.
- Compile data, tracks outcomes, and make recommendations for process improvement.
- Provide referrals/support for follow-up care for patients.
- Travel to multiple locations with the ULH system as needed.
- Graduation from an accredited/approved school of nursing.
- Associate Nursing Degree required – BSN required within 2 years of hire.
- Current RN license in Kentucky.
- 2+ years of oncology experience at the bedside.
- Excellent communication skills and the ability to interact with nurses, physicians, and skilled healthcare professionals.
- Basic Life Support (BLS) certification from the American Heart Association.
- Ability to work autonomously without direct supervision.
- Oncology certified nurse (OCN) recommended or to be obtained within 2 years of employment.
- Academy of Oncology Nurse and Patient Navigator certification recommended or to be obtained within 2 years of employment.
- Previous Cerner experience.
