Empath Home Health, a member of Empath Health, is seeking a PRN Speech Therapist. The Home Health Speech Therapist evaluates, plans, and administers comprehensive speech therapy modalities and regimens as prescribed by a physician to patients with speech, voice, language, cognitive-communicative, and oral-pharyngeal swallowing disorders.Job Title: Speech Therapist - Home Health (PRN)
Location: home health visits to patient homes in Marion, Lake, and Sumter counties
Schedule: PRN shifts available M-F between the hours of 8am and 5pmDUTIES AND RESPONSIBILITIES:
  • Performs accurate and comprehensive initial and ongoing speech, language and swallowing assessments in response to patients’ needs and in accordance with physician orders.
  • Initiates a written plan of care, re-evaluates and updates the plan as necessary.
  • Provides skilled physical therapy modalities within the therapy scope of practice.
  • Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures.
  • Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, to assure patient care needs are met.
  • Addresses and supports patient/family cultural practices, as long as such practices do not harm others or interfere with the planned course medical therapy.
  • Promotes and provides patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic, and/or personal needs or preferences.
  • Supervises Speech Therapy Assistants in compliance with applicable laws, regulations and standards.
  • Plans patient discharge appropriately; provides information about community resources to address patient ongoing needs.
  • Knowledgeable of federal, state regulations and Agency’s policies and procedures regarding patient care.
  • Coordinates patient care and/or services.
  • Communicates with all members of the health care team, including physicians appropriately and timely.
  • Notifies the physician, the Director of Nursing and other health care team members of any changes in the patient’s condition and the need to modify the plan of care.
  • Maintains currency of knowledge and skills.
  • Participates in the Agency’s Quality Improvement program.
  • Attends Agency meetings and in-service programs as required.
  • Obtains appropriate number of continuing education credits to maintain re-licensure status.
  • Maintains confidentiality of patient and Agency information at all times.
QUALIFICATIONS / PROFESSIONAL REQUIREMENTS:
  • Master’s Level Graduate in the area of Speech Therapy or Speech Language Pathology from an accredited university of college.
  • Certificate of Clinical Competence in Speech-Language Pathology awarded by the American speech Language and Hearing Association.
  • For Medicare Certified Agencies:
  • Meets the education and experience requirements for a Certificate of Clinical Competence in speech
pathology or audiology granted by the American Speech-Language-Hearing Association; or * Meets the education requirements for certification and is in the process of accumulating thesupervised experience required for certification.
  • Current valid state Speech Therapy license.
  • Minimum of two (2) years experience as a Speech Therapist.
  • Current CPR card.
  • Current health certificate/physical examination and TB testing results.
  • Valid state driver’s license, reliable automobile and proof of current automobile insurance.
  • Able to communicate effectively in English, both verbally and in writing. Additional languages an asset.
