Job Description:The Clinical Care Management Supervisor is a Registered Nurse responsible for providing oversight to the VBSO clinical care management team. This team includes Care Coordinator RNs and LPNs in Delaware and Pennsylvania. This position will report to the Manager of Care Coordination and be responsible for direct supervision and daily management of the clinical care management team in conjunction with the supervisor of the non-clinical team. The supervisor will assist in developing the clinical care coordination team and performs all functions as outlined in the Care Coordinator RN job description.Essential Functions: * Works collaboratively with VBSO care management teams and leadership to achieve and sustain best practices and staffing targets to support care management activities across the continuum for the populations served.
  • Collaborates with Manager of Care Management and the Non-Clinical Care Management Supervisor to implement and standardize a care coordination program.
  • Assists with maintenance and measurement of the VBSO contract performance standards
  • Participates in interviewing and training of VBSO care management staff
  • Coordinates and provides oversight of VBSO care management new associates to ensure they meet department expectations within 90-day assessment.
  • Serves as a resource to the VBSO care management team to ensure knowledge and understanding of VBSO contracts.
  • Analyzes current processes and data and makes recommendations for process improvements
  • Collaborates with community providers to develop access for patients and improve patient outcomes.
