- Prepare and monitor medications for Kuehner Place and Karin house residents
- provide weekly visits and individual assessments with Kuehner Place residents
- Counsel participants in all Senior Services Programs on healthcare issues
- Support the health needs of participants in all Senior Services Programs as needed
- Conduct community home visits to homebound seniors as needed
- Coordinate care with external providers as needed
- Present educational sessions in the Senior Center on a monthly basis
- Provide healthcare education to all Senior Services Programs
- Provide health, wellness, and medical support to homebound senior patients
- Provide consultation on healthcare issues and support residents in all Senior Services Programs
- Facilitate educational sessions in the Senior Center on a monthly basis
- Maintain case notes for all Senior Service residents; update case notes within 24 hours
- Lead and facilitate health-related workshops twice a month for the senior center participants at Kuehner House and Karin House
- Assist programs with maintaining healthy food options and menu planning
- Accompany and transport clients to medical appointments, as needed
