This includes health promotion/maintenance, counseling, education, assessment, acute/chronic care, and consultation/collaboration with supervising physicians, other health care professionals, and community resources. Exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance. Provides specialized nursing services as needed.Duties:

  • Obtains and records client's health history; performs comprehensive physical assessment as part of a total physical examination. Maintains patient safety goals.
  • Triage patients as needed and provide appropriate care.
  • Diagnoses, treats, and manages reproductive and gynecological illnesses, including performing gynecological procedures: pap smears, breast exams, screening for sexually transmitted infections, colposcopy and insertion of intrauterine devices and implantable devices. Makes referrals as appropriate.
  • Demonstrates skillful knowledge of nursing techniques and procedures, medical and lab equipment and testing procedures, and medication.
  • Orders, conducts, supervises, and interprets diagnostic and laboratory test results.
  • Evaluates and interprets patient data to implement appropriate intervention, creates a care plan, and provide care coordination.
  • Prescribes medication, teaches clients about side effects and monitors responses to prescribed medication, including controlled substances.
  • Provides preventative care, including screening and immunization. Conducts patient education after assessing patient needs.
  • Maintains accurate, legible, and confidential client records and complete medical records in accordance with organizational policy.
  • Participates in staff and organizational meetings as requested for care coordination and development of policies and procedures.
  • Performs additional relevant duties as requested by supervisor.
  • Maintains compliance with all organizational policies and procedures.
  • Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.
Education and/or Experience and Qualifications:
  • Bachelor's degree from an accredited school of nursing: Master’s degree in nursing; completion of a state-acceptable Nurse Practitioner's program; a minimum of one year’s experience is desirable. National certification in a specialty is preferred.
  • Necessary special requirements: Licensed by the State of Oregon as a Registered Nurse, certified by the State of Oregon as a Nurse Practitioner, and Prescriptive privileges.
  • Language Skills: Able to communicate effectively in English, both verbally and in writing; additional languages preferred.
  • Ability to be credentialed with Medicaid, Medicare, private and other insurance.
  • Maintain confidentiality – in accordance with WCHC policies and HIPAA regulations.
Licensure/Certification Requirements:
  • Licensed Nurse Practitioner (Oregon); Fully Credentialed and Insurable.
  • Non-violent Crisis Intervention Comprehensive training class completed within 6 months of hire and recertify prior to the expiration date.
  • Current American Health Association (AHA) Basic Life Support Card (BLS) or within 6 months of hire.
Immunizations Required:
  • TB (test upon hire)
Physical Requirements:
  • Prolonged periods sitting at a desk working at a computer.
  • Able to lift up to 15 pounds occasionally.
  • Able to travel as needed.
  • Exposed to sick people.
